Skilled Nursing
Long-Term Care
Comprehensive long-term skilled nursing.
We know that the hardest thing to accept is that there may be a time when you can no longer continue to provide your loved one with the level of care he or she requires. At Sharp HealthCare, our focus on quality lifetime care focuses not only on your loved one's personal health, but also on their social and emotional well-being. We understand how essential a nurturing environment is to helping each individual retain their maximum level of independence.
Sharp's skilled nursing staff provides your loved one with comprehensive care around the clock. Our dedicated care team is committed to understanding residents' needs, no matter how great or small.
On-site medical director.
Our on-site medical director is available 24 hours every day to assist our residents when needed. Should they require hospitalization, we can provide immediate access through Sharp's comprehensive network of acute-care hospitals.
Personalized nursing care.
When your loved one is admitted to a Sharp skilled nursing facility, the physician is notified promptly so that a comprehensive care plan can be developed. A nurse is available to residents and their families to explain and answer any questions regarding care plans, daily routines and other activities or services available to our residents. Our staff of registered nurses, licensed vocational nurses and certified nursing assistants are available to assist your loved one with whatever care required.
Dietary services.
Our registered dietitians meet with each new resident and their family to review personal dietary needs and preferences in accordance with recommendations from their personal physician. The Dietary Department then will develop a personalized menu plan based on your loved one's dietary needs and personal preferences. Residents are encouraged to dine with others in the Residents' Dining Room. Guest trays also are available for family members or friends who plan a visit during mealtime (advance reservation required).
Recreation and social activities.
We want our residents to lead full lives. That's why we've developed appropriate activity programs based on our residents' individual needs and desires. Arts and crafts, religious services, movies and day trips to restaurants and shopping centers are scheduled periodically to allow our residents to enjoy their favorite activities. Holidays and special occasions also are celebrated - often with a special treat created by our on-site chef. A special festivities room also is provided to allow residents and their families to celebrate special occasions in a private, intimate setting.
Social services.
Our social workers play an important role in your loved one's care. They are there for you. Social workers can explain policies, procedures and facility services to residents and their families. They coordinate individualized care and activity plans and serve as an advocate for your loved one and any concerns or needs your family may have.
Residents' council.
Each of Sharp's skilled nursing facilities provides a residents' council that meets monthly to provide residents and their families with an opportunity to discuss issues or concerns within the facility. Meetings are coordinated by a social worker or activities director but are chaired by a resident. All residents are encouraged to attend these monthly meetings, and residents who are unable to speak for themselves often are represented by family members who attend with them.
Rehabilitation care.
A wide variety of rehabilitation services are available to residents under the direction of their personal physicians. Physical therapy, occupational therapy and speech therapy are available to residents with impaired functional skills resulting from conditions such as stroke, brain injury, spinal cord injury or other neurological impairments. Treatment also is available for conditions resulting from arthritis, orthopedic or cardiac conditions.
