• Health education programs — healthy hearts, tobacco cessation and stress management
• Telephonic health coaching (for patients with a Sharp Rees-Stealy physician only) — one-on-one phone support from a certified health coach
• Dietitian consultations — one-on-one meal guidance for adults and children
